The Community Living Division provides quality services and compassionate support to individuals with disabilities through our Supported Living Program and Group Homes. In the Supported Living Program, we serve a 12 county region in Southern Indiana with 2 central office locations to provide access and convenience to families, guardians and staff.
Support is available 24 hours a day or just a few hours a week depending on the need and wishes of the individual. The supports provided can include anything from assistance with personal hygiene to helping with accessing the community and doing personal errands.
We use a “person-centered” approach and listen to and respect the wishes, needs and desires of the individual, their families and guardians. We believe the premise of our services is simply…Consumer Choice.
We serve all ages, children to the elderly, so we can be with you every step of the way. We work with the following waivers and funding sources:
- Autism Waiver
- Developmental Disabilities Waiver
- Medicaid Waiver
- Support Service Waiver
- Aged and Disabled Waiver
- Traumatic Brain Injury Waiver
We provide the following overall services:
- Behavior Management Services
- Access to professional staff such as: Registered Nurses and Behavior Management Specialists
- Staff hired and extensively trained with the specific individual served in mind
- Person-Centered Planning
- Representative Payee of Benefits
- Self-Advocacy Training
