Develop holistic service plans with various interdisciplinary teams in accordance with agency philosophy and state and federal regulations of the Medicaid Waiver/Supported Living program for consumers on assigned caseload. Implement and monitor service plans, recommending changes and such as needed. Educate staff through regular meetings, direct in-house assistance, and in-service development for staff and volunteers.

Associate’s Degree in the Health and Human Services field or a High School Diploma and a minimum of at least two years writing, monitoring, and implementing plans for individuals with developmental disabilities or similar challenges. Knowledge and experience with Medicaid Waiver program, Case Management, Survey and Audit Process and conducting IDT meetings preferred.
