Implement and monitor service plans, recommending changes and such as needed. Educate staff through regular meetings, direct in-house assistance, and in-service development for staff and volunteers.
Associate’s Degree in the Health and Human Services field or a High School Diploma and a minimum of at least two years writing, monitoring, and implementing plans for individuals with developmental disabilities or similar challenges. Knowledge and experience with Medicaid Waiver program, Case Management, Survey and Audit Process and conducting IDT meetings preferred.

Develop holistic service plans in accordance with agency philosophy and state and federal regulations for group home and waiver/supported living consumers.  Implement and monitor service plans.  Develop and implement objectives designed to promote maximum independence. Educate staff through regular meetings, direct in-house assistance, and in-service development for staff and volunteers. Provide once a  month trainings on Crisis Intervention and QMRP. Provide specified training to new staff during orientation. Provide on-going training to staff as a part of staff development. Organize, schedule, and chair quarterly meetings with the Human Rights Committee to meet program standards.
Bachelor’s Degree in Human Services or related field and at least one year of experience working with individuals with developmental disabilities, as well as Teaching and Training in Educational background or work experience.

Direct Support Professional Team Leader

Our Plainville Group Home currently has a full time 40hr position. To assist the Group Home Coordinator in the daily duties and activities at the group home. To provide one-on-one assistance to residents. To assure that staff vacancies and absences are covered for safety and efficiency by working with schedulers. Monitor and assure the completion of various staff documentation, daily household routines for staff and residents. Perform quality assurance activities to assist with program compliance. Must be highly self motivated, willing to work alone as well as part of the team, and have a positive attitude.      
High school diploma or equivalent. Must be 18 years old and able to obtain/maintain a public passenger/chauffeur license.

Supported Living Quality Assurance Coordinator

Coordinate and provide required orientation, training, and on-going professional development, including observation of staff in home settings, to Direct Support Managers in all Community Living areas to facilitate consistency in application of all policies and procedures. Share emergency on-call duties with other Community Living management staff. Collaborate with the Safety & Statisitical Support Coordinator in facilitating the health and safety committee. Monitor incidents utilizing the Incident Reporting Tracking System. Observe Direct Support managers' hands- on training techniques and activities of direct service staff and provide manager training as needed.
Bachelor's Degree in Human Resources or related field, or High School diploma with knowledge and experience in health and human services teaching and training. Human Resources and or marketing experience helpful, and supervisory experience is necessary. Experience working with people with disabilities preferred.
